The U. S. Food and Drug Administration (FDA) is warning men who have sex with men (MSM) about a possible side effect that could be linked to certain drugs that increase the risk of erectile dysfunction.

One of the drugs used for treating erectile dysfunction in men is sildenafil (also known as Viagra). Sildenafil is available in tablet form, and is used to treat conditions such as high blood pressure and heart failure.

“We will never prescribe Viagra to anyone who’s taking any nitrates (medications) for any form of heart failure or chest pain,” the FDA says.

The FDA recommends men take no more than one Viagra tablet in 24 hours, and those who experience erectile dysfunction or an erection within four hours should see their doctor within 48 hours. Men who have heart failure and high blood pressure should be monitored for the next six months and should be given regular blood pressure and cholesterol tests.

Sildenafil is also used to treat erectile dysfunction in men with other cardiovascular diseases, such as type 2 diabetes and high blood pressure.

Viagra is approved for use in patients with erectile dysfunction. The most common side effects of sildenafil include headache, facial flushing, and nausea.

The FDA notes that sildenafil is associated with an increased risk of death in patients taking nitrates, including nitroglycerin. However, sildenafil does not have an effect on the risk of heart attack or stroke in patients taking riociguat.

The FDA says the side effects are serious and include headache, fatigue, muscle weakness, stomach pain, low blood pressure, and rash. They include abdominal pain and nausea.

“In some men, sildenafil could be associated with a dangerous drop in blood pressure, leading to heart attack or stroke,” the FDA says.

The FDA recommends those who take nitrates for chest pain and have low blood pressure be monitored closely for an erection within four hours of taking sildenafil.

“There is a risk of sudden cardiac death in patients who are taking sildenafil for heart failure and hypertension,” the FDA says.

Some men taking sildenafil may have a higher risk of an abnormal heart rhythm compared with men taking nitrates. The FDA notes that the side effects are not common among men taking riociguat, and some men may experience dizziness, lightheadedness, or fainting. These side effects are more likely in those taking higher doses of sildenafil and in those taking it for erectile dysfunction.

The FDA recommends men not use sildenafil if they have any of the following conditions, including an increased risk of heart attack or stroke.

• A heart attack or stroke that is sudden, severe, or non-arteritic anterior ischemic optic neuropathy (NAION), a condition where blood flows more easily than usual through the optic nerve
• High blood pressure or high blood pressure
• High cholesterol
• Diabetes
• High blood pressure
• Kidney problems
• Stroke
• Pulmonary hypertension

The FDA recommends men taking sildenafil who have had at least two strokes, a heart attack, or a heart attack within the past 90 days.
